Before You Arrive

Give us a call at 928-289-4441 or send a text — we'll get you on the schedule, often same-day or next-day if you need it. Save yourself some time and help us prepare for your visit by filling out our new patient forms below. Or, arrive early to complete them in the office.

When You Walk In

Our friendly front desk team knows the schedule and is watching for you. Once you're checked in, an assistant will take you back, talk through your medical history, and start digital x-rays — they show up on the screen instantly, no waiting. The handheld x-ray we use is a lot less intimidating than the old wall-mounted kind.

Your Conversation with Dr. Gardner

Dr. Gardner will do a full exam, an oral cancer screening, and a periodontal (gum) evaluation, then sit down with you to walk through what he found. If you need treatment, you'll get a written plan with ballpark pricing before anything is scheduled — so you can plan around it. If you just need a regular cleaning, he'll tell you that too.

Patient Forms

Filling out paperwork ahead of time speeds up your check-in. Download what you need below, print at home, and bring it with you. If you'd rather fill them out in the office, that's fine — just plan to arrive 10–15 minutes early.

Your health history, contact information, and insurance details. Also includes permission for treatment, and for us to share information with your insurance carrier.

No Insurance? Our Membership Plan Has You Covered

For patients without dental insurance (or a plan that doesn't cover much), our $299 annual membership makes routine care affordable and predictable — no claim forms, no annual maximums, no waiting periods. You get two cleanings, all necessary x-rays, a fluoride treatment, and an emergency exam visit, plus 20% off any other general dentistry treatment you need throughout the year. It's the simple way to stay on top of your dental care without the surprises that come with insurance.
